Therapy for anxiety

Anxiety doesn't lift because someone tells you to relax. It responds to method — and Cognitive Behavioural Therapy has the largest accumulated clinical research base in this area.

How anxiety actually shows up

Rarely as a dramatic panic attack. More often as background noise: worry that won't switch off at night, a conversation from three years ago reviewed at two in the morning, an email reread five times before sending, an invitation declined because the thought of being there is more tiring than the wish to go.

And there's the body: a tight jaw and shoulders, sleep that doesn't restore, chest tightness, digestive trouble. Plenty of people see a GP and a cardiologist before they see a psychologist.

The cycle that keeps anxiety standing

Anxiety is sustained by a simple, stubborn mechanism. You anticipate a threat; you feel the discomfort; you do something to relieve it — avoid, postpone, check, seek reassurance; relief arrives fast. And it's that relief which teaches the brain the threat was real and that avoiding worked. Short term, you breathe. Long term, safe territory shrinks.

This is why "just stop thinking about it" fails. The problem isn't the isolated thought, it's the cycle. CBT intervenes in the cycle — the anticipating thoughts, the relieving behaviour, and tolerance of the discomfort itself.

What therapy works on

  • Understanding the mechanism. Knowing what's happening in your body, and why, already reduces fear of the symptom itself.
  • Cognitive restructuring. Examining catastrophic predictions: what's the evidence, what happened before, what would genuinely be unbearable.
  • Graded exposure. Moving back towards what's been avoided, in agreed steps, at your pace and never by surprise.
  • Reducing safety behaviours. Identifying the checks and rituals that give immediate relief and feed the cycle.
  • Distress tolerance skills. DBT resources for the moments when emotion peaks.
  • Worry and rumination. Specific techniques for chronic worry, which is a different problem from acute fear.

When anxiety and ADHD come together

A common combination, and the order matters. Someone who has spent a life missing deadlines and forgetting commitments develops, as a consequence, a state of constant vigilance — fear of failing again. In those cases, treating only the anxiety tends to yield little, because the source of the alarm is still running.

What's the difference between normal anxiety and an anxiety disorder?

Anxiety is an expected emotion in the face of threat or uncertainty, and it serves a purpose. It's considered a disorder when it's out of proportion to the situation, persists for months, spreads across areas of life and makes you avoid things that matter. The practical criterion is impairment, not intensity alone.

How long before therapy for anxiety makes a difference?

It varies. Many people report some relief within the first few weeks simply from understanding the mechanism. More durable change, especially where it involves approaching avoided situations, takes longer and depends on the work done between sessions. I don't work with promised timelines.

Does therapy for anxiety work without medication?

In many cases, yes — psychotherapy is indicated on its own for several anxiety presentations. In more severe situations, combining it with medical treatment may be the better option. That assessment is individual, and decisions about medication are always made by a physician.
